New Patient Checklist

It is crucial when choosing a pediatric practice for your children that you have given serious consideration to the fit between the practice philosophies and your own. Excellent care happens when the physician and office staff are aligned with your family priorities and philosophies regarding medical care. Only then can shared decision-making be based on mutual respect.

Please read the following checklist carefully to be sure 
Woodhaven Pediatrics is the appropriate medical home for your children. We look forward to meeting you and forming a mutually rewarding relationship.

  • Vaccines

    We feel strongly that vaccinating children on schedule with currently available vaccines is the right thing to do for all children unless there is a medical contraindication. We require all patients to adhere to the vaccination schedule endorsed by the American Academy of Pediatrics(AAP). Our vaccine policy follows the schedule outlined by the Centers for Disease Control and Prevention(CDC) and the American Academy of Pediatrics(AAP). 

  • Antibiotics

    We work hard to not overuse antibiotics. We educate families on appropriate use of antibiotics, but follow evidence-based guidelines and don’t automatically treat ear pain or a green runny nose with antibiotics.


    We do not routinely prescribe antibiotics over the phone as we do not believe that is good medicine. We will prescribe an antibiotic when we believe it is an appropriate treatment.

  • Medical Home

    We work hard to provide comprehensive medical care and serve as your medical home. We are proud to have carried the designation of Patient Centered Medical Home since 2009. This means that Woodhaven Pediatrics is your home base for medical care. We encourage you to contact our office before seeking specialty care or heading to another provider for urgent care. We want to be involved in either providing care in our office where appropriate, or referring you to the most appropriate specialist and helping to coordinate your care.


    Whenever you do see a specialist, we ask that you request a report be sent directly to our office so we may stay informed and have the most up-to-date information in your medical record.

  • Technology

    Our practice prides itself on efficiency through use of technology. You will be encouraged to consult our website, register for and use our patient portal, and effectively use automated reminders for appointments and for routine care/immunizations that are due. We utilize OP Notify, a HIPAA compliant, secure, texting platform to allow quicker communication with our office both during office hours and after hours.

  • Office Hours

    We make every effort to meet the needs of our patients, offering both routine well visits, sick visit appointments as well as telemedicine appointments.


    We do not offer walk-in appointments. Walk-In appointments will be seen in the first available sick visit appointment time slot. To ensure we provide you with the most efficient care we recommend you call ahead to schedule an appointment.

  • After Hours

    We have a physician on call 24/7/365. When our office is closed overnight and you have an urgent issue that cannot wait until morning, you can page our physician on call by calling our main number and pressing extension 7. We do ask that you allow us some time to return your call, as we do not return calls while driving or during regular office hours.

  • Insurance

    Make sure we participate with your insurance plan. 


    It is your responsibility to know the limits and coverage of your particular health insurance policy, to show your cards to us at each visit, and be prepared to pay any copays at the time of service.


    Our billing staff will do their best to assist you with insurance questions; however, If you have questions about your coverage, it is best check with your specific insurance company.


    Our office does not want you to be surprised by a bill, but must always bill your health plan based on federal guidelines and the actual services provided.

  • Billing / Fees

    Outstanding balances, insurance copays and co-insurance, are expected to be paid at the time of service. If you are unable to comply, you must speak with the billing department prior to the visit to set up a payment plan. As a convenience to our patients and to simplify the billing process, we offer the use of billpay which is an online payment method and also offer to hold a credit card on file.

  • Scheduling Appointments

    For acute care, call the office to schedule an appointment. We will offer same day sick appointments when at all possible.


    If our nurse is with another patient, you will be asked to leave a message and you will receive a return call. We encourage you to utilize our website link "Is your child sick?" for basic illness questions.


    For well visits, please call our office. Well visits may be scheduled up to several months in advance. We encourage you to always schedule your next apointment before leaving the office. You will be reminded in advance of your next scheduled appointment with a mailed postcard.

  • Late Arrivals & Missed Appointments

    To help provide timely service to our patients, we schedule all visits. We will remind you of your upcoming appointment via phone/text message. We make every effort to run on time as we believe everyone’s time is equally valuable. If you are later than 10-15 minutes for your appointment, you will be offered the next available appointment time. Out of courtesy to our other patients, we may ask you to reschedule your appointment.


    Missed Appointments: Missed appointments represent a cost to us, to you, and to other patients who could have been seen in the time set aside for you. We require at a minimum, a same day notice for cancellation of appointments. A fee will be assessed for missed appointments as outlined in our financial policy. Multiple missed appointments will result in discharge from the practice. For new patients, a fee will be charged if the FIRST appointment is missed.

Great news, families — online self-scheduling is officially here! 💻 How It Works Self-scheduling is done directly through our Patient Portal . Simply log in to your portal account to view available appointment options and book online. 🩺 What You Can Schedule Online: ✔️ Well-child exams (when due) ✔️ Sick visits for recent symptoms ✔️ Select follow-up appointments 📌 A Few Helpful Reminders: New patients must call the office to schedule their first visit. Mental health or behavioral appointments (including new ADHD concerns) must be scheduled by phone. Well visits must be at least 1 year + 1 day from the last well exam.  If symptoms have been present for more than 3 weeks , please call the office. We’re excited to offer this convenient new option and make managing your child’s healthcare just a little bit easier. 💙

We’re excited to welcome our new Behavioral Health Care Manager to the team!  Visits with Jennifer require a referral from one of our providers.
